The most common approach to performing this procedure involves a horizontal incision across the lower abdomen — just below the bikini line and just above the pubic area.   It is often performed under twilight anesthesia but if it is combined with the body lift procedures then general anesthesia is required.
   
  It is often combined with liposculpturing and the incisions are placed at the inner thigh crease. Thigh lifts do leave scars. However, thigh lift scars are usually located in a relatively inconspicuous location and, in the minds of many patients, pale in comparison to the benefits provided by the procedure. In many cases, thigh lift scars are easily concealed by underwear or bathing suits. Calf Implants (Calf Augmentation)
 
It is a very popular procedure, which will create cosmetic fullness in the lower leg and can help those men and women who, even after extensive muscular development, can’t achieve the calf muscle toning they desire. Calf implants are best suited for lower leg sculpting, and also can correct muscle imbalance as a result of both physical and congenital defects.
 
Calf implants enhance the shape and size of the calf muscles by placing soft, solid silicone implants in pockets overlying the existing gastrocnemius muscles. The calf muscle is a two-headed muscle (one head on the upper inside of the leg and the other on the upper outside of the leg) on the back of the leg that attaches on each side of the femoral bone just above the knee and extends down the leg ending as the Achilles tendon at the heel.
 
These implants which may be one to each leg or two depending on the desired effect are placed through small incisions in the creases behind the knee. A person may desire to have only the inner head of the muscle enlarged, the outer head, or both. The result is that the existing musculature is made to appear larger and more defined.
 
Procedure is performed under general anaesthetic and requires 4-6 weeks for complete recovery.
   
 
Alternative technique is Fat Transfer
 
Fat cells are harvested from areas of reserve that has excess fat eg. Outer thighs, abdomen and or flanks. The fat cells are processed and injected into the area to be treated.

One may require 2-3 sessions to achieve a desired result. The procedure is performed under local anesthetics.
   
 
Buttock/ Gluteal Enhancement
 
Gluteal Implants, commonly referred to by consumers as buttock or butt implants, are primarily used for augmentation surgery (enlargement of the buttock region) and occasionally for reconstruction or treatment of asymmetry.
 
Buttock implants are usually placed beneath the fascia—or what is termed subfascially—beneath the fibrous lining that encases the buttock muscles. The implants may also be placed in the intra-muscular plane (within the muscle such as the gluteus maximus). The intra-muscular plane of insertion is commonly used when the patient is thin and has little body fat. Both implantation sites are well tolerated by the body.
 
Since this is an area of the body that is required for walking, sitting, and so many other movements, patients might experience greater pain during the recovery period than other common cosmetic surgery procedures and the recovery time is usually longer. Pain medications typically are prescribed to help with the discomfort, but after approximately 5-7 days the patient is able to be up and about; moving, walking, and sitting more comfortably.
 
There is generally very little swelling and bruising after buttock augmentation so the results may be seen very soon after surgery. Full physical activities are usually permitted within a month after surgery.
 
Alternatively
 
Buttock enhancement can be achieved by Liposculpuring.
 
In this technique fat around the buttock region such as flanks, lower back and outer thighs are removed and transferred into more desirable areas of buttock.

 As the years go by, almost every woman notices a gradual deterioration in the size, shape, and firmness of her breasts. In the breast-lift procedure, as we perform it, the incision is a circular one, around the areola with or without a midline vertical incision to the crease. Excess skin is removed from the lower part of the breast, and the nipple, areola, and underlying breast tissue are elevated to a new, higher position. Incisions are closed with very precise, buried sutures that never need to be removed. The procedure takes about two hours, after which a dressing and surgical bra are applied.


 Post-operative discomfort is well controlled with the prescribed medications. Swelling and bruising usually subside within a few days. Most patients are back to work within one week. Most patients will experience some degree of temporary loss of sensation. This will improve with time, as will the scars, which gradually fade.

Breast Reconstruction  


  For most mastectomy patients, the physical loss of a breast is a severe emotional loss as well. A loss of self-esteem; A loss of femininity.


  There are many different approaches for breast reconstruction that vary according to the type of mastectomy that was performed, and to the condition of the breast skin. In some cases, reconstruction can be achieved through a simple implant or prosthesis alone. In other instances, there is not enough skin to accommodate an implant, and a "flap" of donor tissue must be brought into the area. This flap contains skin, muscle, and the blood vessels needed to nourish it and are usually taken from the back or the abdomen to provide adequate skin covering for an implant. In some flap procedures, an implant may not be necessary.


  Another technique involves placement of a balloon-like device called a tissue expander, which is placed beneath the chest muscle and gradually filled with saline to stretch the overlying skin until it is able to accommodate an implant. Additional surgery is often needed to reconstruct the nipple and areola. These reconstructive procedures are usually performed in the hospital under general anesthesia and may require a few days of hospitalization.


  Post-operative discomfort is well managed by medications, and most patients are up and about within one week. Although some scarring is an inevitable part of this procedure, every effort is made to make scars as inconspicuous as possible.


  The newly created breast will not be an exact duplicate of the previous breast nor will it have the same exact contour as the other breast. But the results provide a great improvement and yield considerable emotional and psychological benefits.

Fat Transplantation (Fat Transfer)  



A fat transfer can be used to correct skin irregularities such as deep furrows and wrinkles on the face and other depressions on other areas of the body. This procedure re-contours the face and corrects facial or body deformities.



It is also indicated to enhance body areas such as cheeks, chin, buttocks and calves.



The Procedure For Fat Transfer involves local anaesthesia in both the donor site (area from which fat is taken) and the recipient site (area to be treated. A needle attached to a syringe is inserted onto the donor site in a location which has a thick layer of fat such as the lower stomach, thigh or buttocks. Fat cells, are aspirated or drawn into the syringe are purified and then carefully re-injected into the recipient site. The process is repeated until the desired amount of fat has been transferred and the defect corrected.



There is always some percentage of fat resorption depending upon the areas treated hence 2-3 sessions may be needed to achieve the desired result.

Excessive breast tissue on men is remarkably common. At least one in three have some noticeable excess, affecting either one or both sides of the chest.  Post Operatively

 Irrespective of the procedure used, the chest will be bruised and swollen for several weeks. Pressure garments are recommended to reduce the swelling and bruising, and facilitate healing. Most patients return to their work within one to two weeks and gradually increase their physical activities as the chest becomes more comfortable. However, it may take two to three months before the chest fully settles down and you see the final result.


The results are significant and permanent. Normally, there is a high level of patient satisfaction with reduction of male breasts. Therefore, if your expectations are realistic the chances are good that you will be very satisfied with the procedure and your new look.

The most common approach to performing this procedure involves a horizontal incision across the lower abdomen — just below the bikini line and just above the pubic area.
